Hair Replacement

Approximately 80 percent of men will experience some amount of balding that produces a noticeable change in appearance. Many men, and especially women, become self-conscious as the hair loss becomes more noticeable. While there are several medical conditions that can produce baldness, such as illness, poor nutrition and thyroid imbalance, most baldness is not caused by illness, but is simply an inherited trait.

Fortunately, there are several effective techniques available to correct baldness. These procedures are often combined to produce the best result.

  Hair transplants are the most common procedure for hair replacement. A strip of hair from the back side of the scalp is removed and the donor area closed with dissolvable stitches, leaving a non-visible scar hidden in the hair. The strip of hair-bearing scalp is cut into thousands of small pieces that contain one-to-two, three-to-four and four-hair follicles per piece or graft. Pieces containing one-to-two hair follicles per graft are called micrografts. Pieces containing three-to-four hair follicles per grafts are referred to as minigrafts. These grafts are then closely placed in the bald scalp. The smallest micrografts are placed in front to create a natural-appearing hairline, and the large minigrafts are placed directly behind them to add more hair density.

The procedure is performed in the office using local anesthesia and mild sedation. The procedure takes four hours, and you are able to return home after the procedure. You can return to work the following day. Hair growth starts within three months. More than one session may be necessary to provide sufficient hair density. The transplanted hair follicles are permanent.

Scalp Reduction
Scalp reduction is another treatment for balding. The procedure removes the non-hair bearing scalp on the top and stretches the remaining hair-bearing scalp on the sides to cover the removed scalp. Scalp reduction is used most commonly for balding on the crown of the head and can be used in combination with other hair replacement techniques, such as hair transplants. The resulting scar may be noticeable unless hair transplants are performed to hide them.

This procedure is sometimes performed in two operations. General anesthesia is preferred, and it is generally done as an outpatient.

Scalp Flaps
Scalp flap surgery involves the transfer of large strips of hair-bearing scalp on the sides of the head to an area of scalp without hair, usually the entire frontal hairline. The area from which it is removed is closed with stitches. The hair transferred to the new location continues to grow at the same rate and with the same density as the original location. It is the most efficient way to move the greatest amount of hair in the shortest period of time.

Scalp Tissue Expansion
In men with extensive frontal and crown balding, a modification of the scalp flap technique can produce a dramatic full head of hair. In this two-stage procedure, deflated sacs are placed under the hair-bearing scalp on the sides and back of the head. Once or twice a week the sacs are filled with saltwater in the office until they sufficiently stretch the scalp. The slow, gradual filling of the sacs causes the scalp to stretch and grow, producing enough hair-bearing scalp to completely replace the bald scalp. In the second operation two to three months later, the bald scalp is removed along with the sacs. The redundant, stretched hair-bearing scalp is then rearranged to replace the previous bald area.

Scalp flaps and scalp tissue expansion are usually performed under general anesthesia as an outpatient. Other techniques, such as hair transplantation and scalp reduction, may be combined to achieve the desired result.
Non-Surgical Treatment for Baldness
Two medications are approved by the FDA for treating male-pattern baldness-RogaineTM and PropeciaTM. The modest hair regrowth from these medications is lost when you stop taking the medication.

Synthetic hairpieces are an option for camouflaging baldness. However, these pieces must be replaced periodically.
